C\u2019est dans ce contexte qu\u2019un nouveau domaine a connu un accroissement important en connaissance et en int\u00e9r\u00eat dans les derni\u00e8res ann\u00e9es : l\u2019oxym\u00e9trie oculaire.<\/p>\n<h3>D\u00e9finition<\/h3>\n<p>L\u2019oxym\u00e9trie d\u00e9crit la mesure de la saturation en oxyg\u00e8ne du sang. L\u2019h\u00e9moglobine agit comme un transporteur de l\u2019oxyg\u00e8ne dans le sang. Par cons\u00e9quent, la saturation en oxyg\u00e8ne est mesur\u00e9e comme un rapport de l\u2019h\u00e9moglobine oxyg\u00e9n\u00e9e par rapport \u00e0 l\u2019h\u00e9moglobine totale (1). L\u2019oxym\u00e9trie oculaire mesure sp\u00e9cifiquement la saturation en oxyg\u00e8ne du sang dans la r\u00e9tine. Ce tissu m\u00e9taboliquement tr\u00e8s actif est primordial dans la sant\u00e9 oculaire et par cons\u00e9quent dans le d\u00e9veloppement de pathologies.<\/p>\n<h3>L\u2019\u00e9volution de l\u2019oxym\u00e9trie oculaire<\/h3>\n<p>La premi\u00e8re mesure de la saturation d\u2019oxyg\u00e8ne dans l&rsquo;\u0153il date de 1959, alors que les m\u00e9thodes utilis\u00e9es \u00e9taient plut\u00f4t fastidieuses (2). Ce n\u2019est qu\u2019au cours des deux derni\u00e8res d\u00e9cennies que des oxym\u00e8tres oculaires plus conviviaux ont \u00e9t\u00e9 d\u00e9velopp\u00e9s. L\u2019av\u00e8nement de tels dispositifs a permis des d\u00e9couvertes substantielles dans ce domaine.<\/p>\n<p>Les m\u00e9thodes traditionnelles d\u2019oxym\u00e9trie oculaire utilisent l\u2019analyse de deux longueurs d\u2019onde de lumi\u00e8re, appel\u00e9es points isobestiques et non isobestiques. La longueur d\u2019onde isobestique ne varie pas selon le changement d\u2019\u00e9tat de l\u2019h\u00e9moglobine et sert donc de r\u00e9f\u00e9rence. De son c\u00f4t\u00e9, la longueur d\u2019onde non isobestique est sensible aux changements de saturation en oxyg\u00e8ne du sang dans la r\u00e9tine. Il a \u00e9t\u00e9 sugg\u00e9r\u00e9 que le rapport d\u2019absorption des longueurs d\u2019onde isobestique et non isobestique serait lin\u00e9aire \u00e0 la saturation en oxyg\u00e8ne (3).<\/p>\n<p>Bien que ces technologies aient jou\u00e9 un r\u00f4le important dans l\u2019\u00e9tablissement du potentiel de l\u2019oxym\u00e9trie oculaire dans les soins oculaires, leur manque de pr\u00e9cision a consid\u00e9rablement limit\u00e9 leur utilisation dans les milieux cliniques. Par exemple, ces oxym\u00e8tres traditionnels fournissent des mesures relatives de saturation en oxyg\u00e8ne dans les gros vaisseaux sanguins uniquement. Cette limitation emp\u00eache l\u2019\u00e9valuation de tissus vascularis\u00e9s de structures sp\u00e9cifiques telles que la r\u00e9tine ou la t\u00eate du nerf optique. De plus, ils ne permettent que des mesures ponctuelles et ne peuvent donc pas \u00eatre utilis\u00e9s pour \u00e9valuer des r\u00e9ponses m\u00e9taboliques en temps r\u00e9el.
